Highlands Home Health

Home health care is a wide range of health care services that can be given in your home for an illness or injury. It is usually less expensive, more convenient, and just as effective as care you get in a hospital or skilled nursing health and rehab facility.  Home health is an extension of your doctor’s care or the care you received during a hospital stay. Care may include a variety of therapy and support services to help give you the most independence possible in your home.  

A Highlands Home Health registered nurse will visit your home to do an evaluation of your situation and work with your physician to develop a plan of care to meet your specific needs. We care for patients who see physicians from across North Alabama and beyond, and those who have stayed in many different hospitals.  

Our affiliation with Highlands Medical Center strengthens your continuity of care after a hospitalization through our ongoing communication with the physician, nurses and therapists who cared for you during your stay.  In many cases, if you had a therapist during your stay at HMC, that same therapist will continue your care through Highlands Home Health.  For patients with COPD, the HMC respiratory therapy team will maintain regular communication with you to answer questions, provide education, train you on equipment use and maintenance, and monitor your condition.    

Your assigned case manager, who is also a registered nurse, will supervise your care plan to track progress and help to coordinate the services you need.  All nurses, therapists and support staff stay in close communication with each other and with your physician.  We notify your physician of any changes in your condition or needs, allowing you to focus on your healing and recovery.  

Highlands Home Health is accredited by the Joint Commission on Accreditation of Healthcare Organizations, and treats patients in the Alabama counties of Jackson, DeKalb, Madison and Marshall. We are regulated by federal and state guidelines, and subject to annual reviews.  Highlands Home Health provides services without regard to the race, color, sex, national origin, age or handicap of patients.

Who Qualifies

To qualify for home health under Medicare, you must be home-bound, under the care of a physician, and in need of skilled services on an intermittent basis. Intermittent home care services are those that are only needed a few hours a day.

You do not have to be bed-ridden or totally confined to home to qualify for Medicare reimbursement. You are considered home-bound if you are unable to leave home without considerable and taxing effort on an infrequent basis.
