What is Home Health care?
Home health care is an extension of your doctor’s care and hospital services. It is provided to you in your home. Care may include an array of therapeutic and support services to give the patient maximum independence in the home.

Who qualifies for Home Health under Medicare?
To qualify for home health under Medicare, you must be home-bound, under the care of a physician, and in need of skilled services on an intermittent basis. Intermittent home care services are those that are only needed a few hours a day.

You do not have to be bed-ridden or totally confined to home to qualify for Medicare reimbursement. You are considered home-bound if you are unable to leave home without considerable and taxing effort on an infrequent basis.

Highlands Home Health patient’s plan of care
Highlands Home Health develops a plan of care with your physician, and the plan of care is closely supervised by our clinical staff.  A registered nurse visits your home and develops a plan of care with your physician. The care plan is supervised by your case manager (a registered nurse). All nurses, therapists and other staff stay in close communication with each other and with your doctor and notify the doctor of any changes in your condition or needs.

Highlands Home Health is staffed by trained professionals. All are experienced caregivers with hospital, long-term care or other professional experience. The staff keeps its training up-to-date through continuing education programs on the latest medical skills and techniques.

Highlands Home Health is affiliated with Highlands Medical Center and works closely with your physician to ensure continuity of care.

Although a physician referral is required to initiate home care services, the following people can help you make the necessary arrangements and provide you with further information:

Highlands Home Health treats patients in Jackson, DeKalb, Madison and Marshall counties. Highlands Home Health is accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Regulated by federal and state guidelines, Highlands Home Health is subject to annual reviews.

The Alabama Department of Public Health inspects all home health agencies annually. The inspections take place at the hospital and in patients' homes.
